Care Management guides seniors and families through medical and daily care decisions. We assess needs, build a plan, and coordinate services so care stays organized.
Home care provides hands-on help. Care management creates the plan, coordinates providers, solves problems, and advocates for you across the whole system.
Our services are considered an out-of-pocket expense. Some long-term care insurance companies will reimburse up to a portion of our total bill annually. Medicare and Medicaid do not cover our services.
A family member, legal representative or the client can sign as guarantor for the agreement. The client and guarantor can be one in the same if the client is considered to have legal capacity.
We suggest introducing our services as care advocacy for your loved one. They are leading their plan; we are their advocate. For families with complicated dynamics, stating the fact that bringing in an advocate is beneficial for you as the caregiver and the family unit is also a good approach.
We respect the client’s rights. We seek informed consent whenever possible. If a guardian is in place, we follow the court order and document decisions.
