Case Study
‘Ethel’ fell and broke her hip and was discharged from the hospital to inpatient rehabilitation. Her husband, ‘Norman’, showed signs of memory loss and was struggling to maintain their household on his own while she rehabilitated. Their four adult children lived out of state and were uncertain on next steps. Our Care Manager first met Ethel at the inpatient rehab, reviewed medical records and consulted with therapy staff on her expected discharge home. Then we met with Norman and evaluated their home environment. We provided a comparison of in home services and costs, led a family meeting, and set agreed upon action items. The family chose a plan that included short-term rehab, home safety updates, and in home caregiver support. We monitored progress and shared regular updates with the four children. Stress dropped and ‘Ethel’ and ‘Norman’ were able to reunite, living together in their home with supportive care.
