Case Study
‘Maude’ called after her father’s hospital stay in Athens. Ever since he was discharged back to her home, he had felt weak and they had difficulty getting to his follow up medical appointments. ‘Maude’ was concerned that he wasn’t recovering as quickly as she hoped. ‘Maude’ engaged our caregiver coaching services to help her understand what services were available for them in the home and to learn how to better care for him as he was requiring more hands on support. Within four weeks of caregiver coaching ‘Maude’ had implemented a small team of support to help her care for her father as he continued to recover. She designed and implemented a care plan with a medication management system that enabled better communication and medication compliance. This ultimately helped better manage his pain and allowed him to be able to participate in home therapy services. This planning provided the supports needed for a smoother recovery and Maude reported less stress and better sleep.
